PHARMACODYNAMICS
Glutathione is an endogenous tripeptide and is present in the cells of all organs and systems of the body. This widespread presence is related to the diversity of glutathione's biological functions; including its important roles in many biochemical and metabolic processes.
The sulfridyl group of cysteine in glutathione is highly nucleophilic and therefore readily reacts with chemicals or other metabolites in an electrophilic manner, resulting in the inactivation of potentially toxic xenobiotics. Furthermore, reduced glutathione, when reacting with a large number of metabolites via oxidation, forms less toxic complexes that can be readily metabolized and excreted as mercaptan acids.
Therefore, glutathione can be applied in the treatment of poisoning related to the above mechanisms, such as liver poisoning caused by ethyl alcohol or drugs, or by specific chemotherapeutic agents...
PHARMACOKINETICS
After intravenous administration, glutathione is mainly contained in red blood cells, while in plasma it is rapidly degraded by gamma-glutamyl-transpeptidase and gamma-glutamyl-cyclotransferase. Therefore, the peak concentration of reduced glutathione in plasma is very small, even at high doses (peak plasma concentration is only about 1 nmol/ml after 600 mg intravenously); while the extent of cysteine metabolism is much larger (peak plasma concentration is 17 nmol/ml). The opposite is true for blood concentrations, which reach about 100 nmol/ml within 5-10 minutes of intravenous administration of 600 mg glutathione. Blood concentrations then gradually decrease and reach a steady state 60 minutes after the drug is administered.
Dosage - How to use
– Intravenous infusion: 600 mg/day.
1. Use by intravenous infusion:
1.1. Helps reduce the neurotoxicity of radiation therapy and cancer chemotherapy:
+ Slow intravenous infusion of glutathione immediately before radiotherapy 15 minutes: Dosage 1200 mg.
+ Slow intravenous infusion of glutathione for 15 minutes before chemotherapy regimen of chemicals (cisplatin, cyclophosphamide, oxaplatin, 5 fluorouracil, carboplatin): Dosage 1500 mg - 2400 mg. Repeat dose 900 mg - 1200 mg after the 2nd and 5th day of treatment. Can repeat weekly dose 1200 mg.
1.2. Support in the treatment of mercury poisoning: Combining specific drugs to treat mercury poisoning such as 2,3 - dimercaptopropan - 1- sulfonate and meso - 1,3 - dimercaptosuccinic acid with glutathione infusion and high doses of vitamin C reduces blood mercury concentration. Acute dose is 1200 - 1800 mg/day. Maintenance dose is 600 mg/day until recovery.
1.3. Support in the treatment of alcoholic cirrhosis, cirrhosis, viral hepatitis B, C, D and fatty liver:
+ Support treatment of alcoholic cirrhosis: Dosage 600 mg - 1200 mg/day, slow intravenous injection.
+ Support treatment of cirrhosis, viral hepatitis B, C, D and fatty liver: 600 mg - 1200 mg/day, slow intravenous injection until recovery.
1.4. Supportive treatment in diseases related to peripheral vascular disorders, coronary artery disorders and hematological disorders:
- Peripheral vascular disorders: 600 mg/time, 2 times/day, intravenous infusion.
- Coronary artery disease: intravenous infusion 1200 mg - 3000 mg or direct infusion into the left coronary artery 300 mg (50 mg - 2 mL/minute).
- Patients on dialysis due to chronic renal failure: Infusion of glutathione 1200 mg/day at the end of each dialysis cycle helps reduce the dose of erythropoietin by 50%.
1.5. Supportive treatment of subarachnoid hemorrhage: Slow intravenous infusion of 600 mg glutathione immediately after surgery, repeat the above dose every 6 hours for about 14 days or more.
1.6. Support in the treatment of non-insulin dependent diabetes: 600 mg - 1200 mg/day, slow intravenous injection continuously for one week, then use 2 - 3 times per week, 0.6 g each time.
1.7. Support in the treatment of acute pancreatitis: 600 mg - 1200 mg/day, slow intravenous injection.
2. Use by intramuscular injection:
Support in the treatment of male infertility: 600 mg -1200 mg/day, intramuscular injection continuously for 2 months.
Attention:
The reconstituted solution for injection is stable for about 2 hours at room temperature (25°C) and about 8 hours at 0°C to 5°C.
Instructions for use:
Intravenous infusion: Reconstitute the 1200 mg powder vial with 4 ml of sterile water for injection, then dilute with at least 20 ml of infusion solution: 5% dextrose, 10% dextrose, 0.9% sodium chloride, Lactated Ringer, 1.4% sodium bicarbonate, etc. Intravenous infusion for 30 minutes
Contraindications:
Hypersensitivity to glutathione or any of the other ingredients of the drug.
